Agreement to Participate in the PCPL x White Lotus Lion + Dragon Dance Juniors Program
ACKNOWLEDGMENT OF RISK AND RELEASE OF LIABILITY
I, the undersigned, acknowledge that participation in lion dance classes, rehearsals, and performances involves inherent risks, including but not limited to physical exertion, falls, collisions, muscle strains, and other potential injuries.
I voluntarily assume all risks associated with my child’s participation in dance activities conducted by PCPL x White Lotus Lion + Dragon Dance and its instructors.
WAIVER AND RELEASE
In exchange for the opportunity to participate in dance classes, rehearsals, performances, and related activities with PCPL x White Lotus Lion + Dragon Dance, I, the undersigned, willingly agree to the following terms:
Release and hold harmless PCPL x White Lotus Lion + Dragon Dance, its owners, instructors, employees, and affiliates from any claims, liabilities, damages, or expenses arising from my child’s participation in dance activities.
Accept full responsibility for my child’s safety and well-being while attending classes and related events.
Disclose any pre-existing medical conditions that may affect my child’s ability to participate safely. I understand that it is my responsibility to seek medical advice if I have concerns about my child’s physical condition. I agree to inform the instructor of any conditions that may affect participation or safety during the class.
MEDICAL RELEASE
In the event of an injury or emergency during class, I hereby authorize PCPL x White Lotus Lion + Dragon Dance and its representatives to seek appropriate medical treatment on my behalf. I understand that I am responsible for all expenses incurred as a result of such treatment. If you have a preferred ER, please note it below.
MINOR PARTICIPANTS
I, the parent/guardian of the Minor listed below, acknowledge the risks involved in dance activities and consent to their participation. I agree to the terms of this waiver on behalf of my child.
